Palliative Care

Comprehensive palliative care services are provided at the James J. Peters Medical Center through an interdisciplinary team.

Consults may be requested for any patient with an advanced, life-threatening or terminal illness and can include such concerns as:
  • pain management
  • family support
  • dyspnea
  • appetite
  • constipation
  • nausea/vomiting
  • disposition planning
  • supportive counseling
  • goals of care/advance care planning
Supportive Care Unit
  • Description: Inpatient unit located in the Nursing Home Care Unit. Offers care focused on comfort including expert symptom management and psycho-social-spiritual support to veterans and their family members at the end of life. This unit is appropriate for veterans with a life expectancy of less than 6 months who are not pursuing life-prolonging therapies and who can not be managed at home.
  • Services: Veterans are followed daily by the Palliative Care Team who serve as primary care providers. Veterans are usually housed in private rooms which have a refrigerator and sofa bed to accommodate family members. Veterans and their families are offered ongoing supportive services from Social Work, Psychology and Chaplaincy services. Nutrition, wound care, recreational, and rehabilitative services are offered as appropriate.
  • Process: Referral from inside the medical center is made via a Palliative Care Consult.

Palliative Care Clinic
  • Description: Outpatient palliative care services are offered to all veterans with a life-limiting illness who require symptom management, psycho-social support or assistance with advance care planning. The clinic meets weekly in 3C and is available by appointment only.
  • Services: The Palliative Care clinic is staffed by a physician board certified in Palliative Care as well as a Social Worker. Management of symptoms related to a life-limiting illness, advance care planning and psychosocial support are provided to veterans and their families.
  • Process: Referral from inside the medical center is made via a Palliative Care Outpatient Consult.

Pain Management
  • 90% of cancer pain can be adequately managed using the WHO analgesic ladder.
  • In treating chronic pain, prevention is the goal. Medications should be given around the clock, not PRN and the oral administration route is preferred (WHO Guidelines).
  • Any patient receiving chronic opiates must be placed on a bowel regimen.
  • All patients on standing opiates should have a rescue dose available, which is usually 10% of the daily standing opiate dosage.
  • Palliative Care and Pain Management are available for consultation if there are any questions related to opiate dosing. Methadone and Transdermal fentynal should only be initiated after consultation with Pain Management or Palliative Care.
  • Due to toxic CNS metabolites, Meperidine should not be used for management of chronic pain or in high doses.
